Required Information for DAHS Authorizations

As a reminder, Day Activity and Health Services (DAHS) facilities requesting authorization for DAHS services have several options. For initial and transfer requests, they can submit:

1. Either set of forms designated for requesting DAHS services by the Texas Department of Aging and Disability Services (DADS);  OR
2. Other documentation (current to within the last 30 calendar days) that includes specific required elements.

For renewal requests, providers also have the option to submit the last three months of monthly nursing assessment notes completed by the DAHS facility nurse, in lieu of the documentation indicated above.

Please note: When submitting any request for DAHS authorization, providers must include a diagnosis and current medication list.
