Requesting Prior Authorization for DME Supplies

As a reminder, prior authorization for Durable Medical Equipment (DME) supplies less than $500 is no longer required for claims with dates of service from December 1, 2017 and forward (except when specified on the Superior HealthPlan Prior Authorization List). 

For DME supplies less than $500 that do not require prior authorization, the member opt out form is not applicable and, therefore, not required. The designated opt-out fax number should no longer be utilized.

Incontinence supplies under $500 will still require prior authorization, unless they are ordered through Superior’s preferred DME supplier, Medline, and appear on the Medline supply list of items that do not require authorization.
