New: Monogram Program for Members with Chronic Health Conditions

Effective July 1, 2026 Superior HealthPlan is partnering with Monogram Health to provide extra care for members with multiple chronic conditions. All eligible Superior HealthPlan members who meet the Monogram program criteria can get in-home treatment and care for multiple chronic conditions.
Monogram is the leading provider of in-home care for members with multiple chronic conditions. Monogram takes a comprehensive and personalized approach to a person’s health. They evaluate and treat the whole person and all chronic conditions that they may have. This includes:
- Diabetes
- Hypertension
- Chronic kidney disease
- Heart failure
- Depression
- Chronic obstructive pulmonary disease
- And more
Monogram has a large clinical team, with many kinds of specialists. This includes:
- Nephrologists
- Cardiologists
- Endocrinologists
- Pulmonologists
- Behavioral health
- Palliative or end of life care
These specialists help diagnose and treat chronic conditions. They can review and prescribe medication. They can also provide education and guidance on your healthcare options. Additionally, they can help with access to healthy food, transportation, financial help and more.
