Manage Ongoing Conditions with Our Disease Management Programs

Superior HealthPlan offers Disease Management programs* to help you take action to manage your ongoing conditions or healthcare needs. Our goal is to help you become a better, more informed and more active manager of your health. These programs work because you don’t do it alone. You will be matched with your very own Care Manager or health coach. Our healthcare team is ready to work with you to set and reach your health goals.

Superior offers the following Disease Management programs:

Behavioral Health Program – Sometimes experiences like being taken from home and placed in foster care can be stressful and hard to manage. Most children can adjust over time. For some children, emotional, behavioral or developmental problems can happen. This program can help with conditions like attention deficit hyperactivity disorder (ADHD) and depression. If you need help with a behavioral health problem, we have a group of mental health and substance use disorder specialists who can help. Superior offers a 24/7/365 Behavioral Health Crisis Line for member and caregiver support. To access the Behavioral Health Crisis Hotline call 1-866-912-6283, press * (star).

Diabetes Program – Our diabetes program  assists members with Type 1 and Type 2 Diabetes by providing self-management strategies designed to reduce the development and progression of diabetic complications.

 

Lifestyle Program - The lifestyle program provides telephonic outreach, education, and support services to members in support of changing unhealthy behaviors and motiving members on risk reduction and achievement of health goals. Health coaches are available to assist with personal goals like stopping smoking, becoming more active, eating healthier, and reducing stress.

Respiratory Program - A disease management program to help members manage chronic respiratory (breathing) conditions like asthma. With tips on how to identify triggers, manage symptoms, and develop coping strategies, the program connects members to a registered or certified respiratory therapist to help you manage their symptoms.
