HHS Requirement: CCP Private Duty Nursing Prior Authorization Form for STAR Health and STAR Kids

Effective September 1, 2020, providers must submit the Texas Medicaid & Healthcare Partnership (TMHP) Comprehensive Care Program (CCP) Prior Authorization Private Duty Nursing (PDN) 6-month Authorization Form.

In order to comply with Texas Health and Human Services (HHS) requirements and to align with the September 2020 update of the STAR Kids Handbook, Superior will implement the TMHP CCP Prior Authorization Private Duty Nursing 6-month Authorization Form. This form must be submitted by providers requesting a 6-month PDN authorization for STAR Health and STAR Kids members. This requirement becomes effective September 1, 2020.
