HEALTH PASSPORT: New Upload Feature

Superior HealthPlan has added new upload options to the Assessments module in Health Passport. This module allows providers to upload Texas Health Steps, COVID-19, and now Behavioral Health documents directly online, including a PDF copy of a member’s Initial Behavioral Health Assessment or Monthly Behavioral Health Assessment form.

To begin using this new feature:

  • Log into Health Passport, to access the Assessments module in the left navigation bar.
  • Once in the module, select the Upload tab.
  • Then, select the Document Type, which now includes an option for an Initial Behavioral Health Assessment or Monthly Behavioral Health Assessment form.
