On April 22, 2024, UnitedHealth Group issued a press release, providing an update on the Change Healthcare cybersecurity incident that occurred on Feb. 21, 2024. Given the size of the data impacted, the investigation to determine whose data is impacted is expected to take several months. UnitedHealth Group believes this situation will impact “a substantial proportion of people in America” and is offering immediate credit monitoring and identity protection services, as well as a dedicated contact center to address questions. What Is Needed on an H2067 Form?

As part of Superior’s continued efforts to improve Superior’s authorization process and expedite the turn-around time for completing and submitting STAR+PLUS long-term services and supports (LTSS) authorizations, Superior would like to request that LTSS providers include NPI and TIN ID numbers on every H2067 form submitted to Superior.

Including these two identifiers on the form will allow Superior Service Coordinators and Program Coordinators to process authorizations quickly and efficiently, as well as easily identify the provider for each submission. For providers, this will significantly reduce the turn-around time for your authorizations.
